Travel Consent Form
You can authorize a child (minor) to travel with one parent (internationally) or another adult (i.e. mother, father, grandparent, aunt, uncle, etc.) domestically or internationally using a Travel Consent Form (Letter). The Travel Consent Form is also known as the Child Travel Consent Form, the Parental Travel Consent Form, the Parental Consent Form for Travel, or the Consent to Travel Form.
An Apostilled Birth Certificate
The Travel Consent Form could be required if the child's Birth Certificate lists two parents. If a parent has sole custody provide the original (or notarized true copy)of the Custody Order or Judgment awarding the sole custody.
An Apostilled Divorce Certificate
The Travel Consent Form could be required even if the divorce documents award custody of the child to the accompanying parent, but the non-custodial parent has legal access or visiting rights to the child.
An Apostilled Death Certificate
The Travel Consent Form could be required if a parent is deceased.
Apostilled Marriage Certificate, Adoption Documents, Certificate of Name Change, etc.
The Travel Consent Form could be required if a parent's last name (surname) is different from the child's last name. Proof of the relationship may be required. This is recommended even if the child will be carrying a passport identifying the parent.
You can obtain a Travel Consent Form either online, from a travel agency or from an airline, cruise ship, etc. The form should include:
- Your name
- The child's name
- The name(s) of the adults traveling with your child
- The country the child will travel to
- A statement, allowing one or more adults to take your child out of the country.
- Travel dates
- Return date
- Airline Information
- Passport numbers
Before a Travel Consent Form can receive the State Apostille, it must be signed and notarized. The signer of the document must physically appear before a Notary Public. Be sure to use Notary from the state where you reside in, who will know the notarial laws in your state. Once notarized, the document can then be mailed to us for processing. A Travel Consent Form can only be Apostilled in the State the document was signed and notarized in.
Once the English Travel Consent Form is properly prepared you can mail them to us to be Apostilled. We can also provide Certified Translation Services into the language required by the Country of Designation.
